Primary Health care represents the first point of contact for individuals with health care system, and is the key to efficient, timely, quality family and community care based on continuity and coordination, early detection and action, and better information on needs and outcomes.

Do You Need Primary Care Provider (Family Doctor)? 

If you need primary care provider, we are here to help. Health Care Connect is designed to help Ontarians who are without a family health care provider (family doctor or nurse practitioner) to find one. People without a family health care provider are referred to a family doctor or a nurse practitioner who is accepting new patients in their community.

How can I Register with the program?

You can register with Health Care Connect if you do not already have a family doctor.
