Service Description:
|
Provides outpatient care, education and support to children and adults with type 1 and 2 diabetes, prediabetes and gestational diabetes * team consists of diabetes educators (nurse educators and dietitian educators), social worker, kinesiologist (exercise specialist), endocrinologists, nurse practitioner, and support staff * supports patients learning knowledge and skill on diabetes management and staying on track with healthy living behaviours * education is provided through individual appointments, group programs, and workshops at a variety of locations in the Halton region
Adult Education Program- one-to-one teaching and group sessions
- initial individual assessment
- follow-up visits
- one-to-one appointment a Nurse Practitioner in Endocrinology or an endocrinologist
- all types of diabetes and prediabetes

Primary care providers (family doctor or nurse practitioner) or community endocrinologists can refer a patient by contacting the Mississauga Halton Central Intake Program at 905-338-2983 ext 4872 * adults interested in attending a Diabetes Education Program can also contact the Mississauga Halton central Intake Program to initiate the referral process (self-referral)
