Service Description:

Community-based health care * paramedics operate in an expanded role, outside traditional emergency response and transport * paramedics work collaboratively with primary care providers, Ontario Health atHome, hospitals, and community support services to help people access resources and remain healthy and safe at home
Services include:- home visits
- remote care monitoring
- file of life
- referrals to community resources
- CP@Clinic
- public access defibrillator (PAD) program
