Service Description: Community-based health care * paramedics operate in an expanded role, outside traditional emergency response and transport * paramedics work collaboratively with primary care providers, Ontario Health atHome, hospitals, and community support services to help people access resources and remain healthy and safe at home

  • file of life
  • referrals to community resources
  • CP@Clinic
  • public access defibrillator (PAD) program
